A Brief History of Upset Training
On March 3, 1991, a Boeing 737 crashed near the approach end of Colorado Springs Municipal Airport runway 35. The cause of the crash was later determined to be loss of control in-flight (LOC-I) following a rare rudder malfunction. United Airlines Flight 585 struck Widefield Park at a nearly vertical attitude, killing all 25 people on board. Two additional B-737s suffered the same malfunction that decade. In the third event, Eastwind Flight 517, the pilot was able to recover and land safely. The National Transportation Safety Board released its related safety recommendation and pointed to a longstanding concern about additional training. Is that true? Was that concern evident?.... Emergency Response Simulation Drill Planned for NBAA2015
Is your flight department prepared in the event of an emergency? Have you ever conducted an emergency preparedness drill with your frontline personnel? It is important for schedulers and dispatchers, who are often considered "frontline" personnel, to practice your organization's emergency response procedures under realistic conditions. Don Chupp, President of Fireside Partners Inc. worked with the NBAA Schedulers & Dispatchers Committee for nearly a year to bring a live emergency preparedness drill to the 2015 Schedulers & Dispatchers Conference. NTSB Releases Agenda and Call for Questions for General Aviation Safety Forum
The National Transportation Board has released the agenda and participants for its forum titled Humans and Hardware: Preventing General Aviation Inflight Loss of Control. The event will be held on October 14 in Washington, D.C. ACSF's Safety Program Now Covers All of Contiguous U.S.
The FAA's Central and Northwest Mountain Regions have signed a memorandum of understanding (MOU) allowing charter operators, corporate flight departments and fractional program managers to participate in the Air Charter Safety Foundation's Aviation Safety Action Program (ASAP). This declares the ACSF-managed ASAP program now approved in the contiguous U.S. ASAP, an FAA/industry partnership, provides a near consequence-free environment in which participants can identify and report safety issues that could be critical in identifying potential precursors to accidents....
